Patient and Family Centered Care Program at the U-M Frankel Cardiovascular Center

At the heart of the U-M Cardiovascular Center lies the belief that providing the ideal care experience stems from a partnership between patients, their families, physicians and staff. CVC has created a Patient and Family Centered Care Program, to provide a forum for patients and families to share their personal experiences and partner with the CVC faculty and staff to improve care.

Patient and Family Centered Care has four Core Concepts:

• Participation
